Online RE form

          

To access and complete a RE Form, please click here.

 

Important Information:

• To access the RE Form, enter your User Name (which starts with a capital E, followed by your employee number without the lead « 1» and zeroes, ex. 100010568 = E10568) and Password (which is the same as your InfoWeb password, all in capitals). For assistance with login, please contact the Computing Help Centre online at www.ccs.uottawa.ca/cybersos or by telephone (613) 562-5800, extension 6555.
  
  
• Once you have entered the RE Form portal, you can:
  
  
  • Create a new RE Form;
    
  • Continue completing a RE Form already started and;
    
  • Consult RE forms already submitted.
    
    
• For every new request for funding, you must complete a RE Form.
  
  
  • Click on ‘Create new RE Form’.
    
  • You must read and accept the terms and conditions related to grants and contracts administered by the University of Ottawa. Once you do so, you will access the RE Form.
    Once completed, you click ‘Submit’ at the bottom of the form to submit your information to Research Management Services (RMS) for revision and approval. You can also select ‘Save as Draft’ to save the form and come back to it at a later time.
    
  • Once submitted, you must print a copy of the RE Form and obtain the required signatures. This signed form must be forwarded to RMS. Without this signed RE Form, RMS cannot approve your request. Please note that you will receive all relevant instructions as you complete and submit the RE Form.
    
    
• To continue completing a RE Form already started and to consult RE forms already submitted:
  
  
  • Click on the RE number of the said form to continue completing it or to consult it.
    
    
• A Reference Guide is also available on the RE Form portal (simply click ‘Reference Guide’) to assist you as you navigate through the form. You will also find that guide here.
